New to prostate cancer

Hi All. Just got fused trans perennial biopsy results back, and 5 of 12 cores are positive. Found on both sides. The worst is 2 cores at 70% with Gleason 3+4 but Gleason 4 is less than 5. Other 3 at 3+3 with the highest at 30% of the core.

PSA and family history

By the way, MRI results indicated no clinically significant focal lesions were seen. How can that be with 5 cores positive? At a loss there. 58 years old and PSA jumped to 4.5 in one year after mid-2s first 3 years of tests starting at 55 ish. Mother's brother had it at 55 and went radicle to get it out. 11 years later no sign of PSA still. Results online but with all early research well prepared for euro appt in a couple of days. Will be interesting to hear feedback but assume still some testing needed to make sure no spread first.
Seems at my age and potential life expectancy may have to go radicle and surgery
