Prostate MRI T3

Hi Friends,
I just recieved my MRI result which was done because of a rising PSA. The result was category 5 which is a clinically significant cancer probability, according to the report. But I also read that other conditions can result in a highlighted signal intensity, thoughts on this? Is it worth it to look for a second reading of the report?
I can also upload my report, but wanst sure if its advisable
thank you
Patrick
PS if it is advisable, do you have any recommendations that will review the report and disc?

  1. Hi . Sorry to hear about the PI-RADS 5 finding from the MRI. First, I want to note that we are not medical professionals and, for your safety, cannot offer medical advice. That said, PI-RADS 5 is the highest level on the scale. As noted in this article, the PI-RADS scoring has limitations, but it is largely in the realm of the 3 and 4 scores that questions arise in the evaluation: https://www.nature.com/articles. This is not to say that a PI-RADS 5 is always cancer, but, unfortunately, there is no way of knowing without a biopsy. This article goes into further detail on the scoring process: https://cancerimagingjournal.biomedcentral.com/articles/10.1186/s40644-016-0068-2. Your doctor should be able to provide further guidance on the report and next steps. Wishing you the best and please feel free to keep us posted on how you are doing and to ask additional questions. 1. Hi . If you are looking for percentages for PI-RADs scores that end up being cancer this research found that "For PI-RADS category 3, 4 and 5 lesions, the overall proportion of cancers was 22.2 percent (78 of 352), 39.1 percent (43 of 110), and 87.8 percent (129 of 147), respectively:" https://www.rsna.org/news/2018/september/pi-rads-prostate-cancer-diagnosis#:~:text=For%20PI%2DRADS%20category%203,129%20of%20147)%2C%20respectively. For your protection, we cannot give medical advice over the internet. Each case is different and the decision on how to proceed has to be yours. If you are concerned about the risks of biopsy you could discuss urinary biomarker testing with your doctor. Best, Richard (ProstateCancer.net Team)

    2. Thanks Richard for the Biomarker information!

  2. I had two lesions. PIRADS-5 and PIRADS-4. The PIRADS-5 was Gleason 3+4. The PIRADS-4 was benign so I basically fit within the percentages posted. The results of a Pylarify PSMA PET Scan are going to generally be more accurate than a 3T MP MRI with tracer as far as telling you the chance that you have cancer, in my opinion. Either way, my opinion is, with a PIRADS-5 MRI, having a biopsy done is no-brainer. It was far easier of a procedure than I thought it would be.
