I recently doctored in one state where they did my MRI after biopsies and it did not show the cancer - but I was very swollen. I did not like various things that had happened - including traumatic biopsy experience - so I went to Mayo Clinic. At this point former doctor had me at Gleason 6. PSA just over 5. New doctor did new MRI. Scan was PIRADS 5. Extracapsular extension with fat stranding. PSA is 7.1. Robotic surgery on Friday. I do not have new Gleason score - but said may be 8. Old doctor was basically shooting air on biopsies. My lesion is 1.6 cm. What are your insights? Just how bad is this?

  1. Hi . Glad you took matters into your own hands and got a second opinion. For your protection, we cannot give medical advice over the internet. What I can say is that, while Pi Rads 5 is considered very suspicious and and the extracapsular extension illustrates the potential for breakout, it sounds like your cancer may still be contained or in nearby tissues. Has your doctor said what he thinks the tests say regarding spread? As you know, the Mayo clinic is one of the finest medical centers. I can also tell you that we have contributors here who had Gleason 9 and now have undetectable PSA. Hopefully others will chime in with their experiences.     1. I am one of those who had a Gleason 9 and yes so far I am undetectable. My psa was 2.3 when diagnosed in 2013 it was verified my cancer was contained in the prostate. I did undergo surgery and no spread was seen. 5 years later it did come back and i underwent radiation and hormone therapy. We are in 2022 and ok so far. My work has been done at Hartford Hospital which partners with Memorial Sloan Kettering on prostate cancer treatments and others . You are wise to be involve with a major hospital. I will say a Gleason 6 is generally rated as less likely to spread and 8 is a different situation - Hope you do well with your surgery

          1. Agree Mayo is one of the best - A bud of mine also found them to be excellent in handling his prostate cancer
