First PSA post HDR Brachy... 4.6

October 2022, I was diagnosed with Gleason 3+4 prostate cancer. I had the multiparametric MRI and fusion biopsy. Two months later I had the F18 PSMA PET/CT scan, confirming it was confined to the prostate and not yet metastisized. PSA had risen from about 5.2, up to 13.4 over the course of a year.

I had HDR Brachytherapy Mar. 7, 2023. It was two sessions over two consecutive days.

I had my first PSA test, 3 months after the procedure and was expecting it to be low. The test was 4.6 and I'm very worried that the Brachytherapy did not work as intended.

Have any of you had similar outcomes and if so, what was done about it?

  1. Hello Paul, I feel your anxiety and concern…I was diagnosed with PC in Jan 2022, and had same reading as you 3+4. I decided with External Radiation therapy (no ADT), 20 session over a month. The PSA prior to radiation was12.8. The post radiation PSA readings were 7.04, 5.75, 4.03, 3.21 and the last one almost a year after radiation was 1.84.


    So as you see it took almost a year to get to this result, and my urologist and. Oncologist are saying as long as it comes down it is good news and at one point PSA should stay at its plateau level which will be the nadir value. And the nadir value not necessary will be same for each individual.


    So I’m sure you already discussing your result with your doctor. But in my none medical opinion it seems your PSA has gone done significantly in first reading. Also any treatment other than complete removal of prostate and combination of hormone therapy (ADT), the PSA reading will fall gradually.
