PSA Has Just Increased Slightly 16 Years After Prostatectomy

Hello, Had a prostatectomy in 2006 at age 53. Gleason score was 6. PSA pre-surgery never higher than 1.1. Pathologic stage: pT3a N0 MX R0. Tumor had breeched capsule on one side but margins were clean as were lymph nodes. Have had PSA tested annually since then and it always came in at 0.01. However, just had my annual PSA yesterday and the number has increased slightly to 0.04 at age 69. Should I be concerned? Or just continue checking it annually and reach out to my urologist if/when it exceeds .2 or .4? Thanks.

    I have a similar story. My RP was in 2007 at the age of 47. Gleason 6, clean margins - everything great. I went 15 years with undetectable PSA until August 2022 at age 62 - the PSA has risen to 0.05. My urologist ordered a PSMA PET which came back negative. Best of luck to you in your journey!

      1. Hi . Bringing the urologist into the loop whenever there is an increase in the PSA is never a bad idea. That said, the gold standard on finding recurrent prostate cancer is the PSMA PET scan (see: https://prostatecancer.net/clinical/psma-imaging-scan) and it generally is considered effective at PSA levels of 0.2 or greater.
