Gleason of 6

Irritated pelvic and groins. Hello everyone. Any information will be greatly appreciated. For about three weeks I've had those uncomfortable feelings. Urination urgency and frequency for about 6 months. My urologist has put me on three different medications for OAB but so far no relief. I'm really worried now that the irritation and discomfort in the pelvic area groins could symptoms of the prostate cancer. I'm supposed to see a urologist outside the VA in November to discuss robotic surgery. Your opinions,advices would be greatly appreciated. Thank you

  1. I meant symtoms of prostate cancer spreading. My last psa in October 2025 was 2. 69. Thank you

  2. Hi . It is certainly understandable that you would be concerned about these new symptoms. You may recall that I earlier shared information on the debate on whether Gleason 6 should be considered cancer. Part of that is due to the fact that Gleason 6 does not metastasize (see here for example: https://www.urotoday.com/conference-highlights/aua-2023/aua-2023-prostate-cancer/144054-aua-2023-debate-renaming-gleason-6-prostate-cancer.html). There are things related to overactive bladder and enlarged prostate that can cause pelvic and groin discomfort, such as prostatitis (see: https://www.mayoclinic.org/diseases-conditions/prostatitis/symptoms-causes/syc-20355766) or BPH resulting in pressure on the pelvic and groin region. Of course, only a doctor can provide the proper diagnostics and it is important to ask your doctor about any possibility of a higher Gleason score.
