Worried prostate cancer has spread

My dad had a ct scan which showed he has a mildly enlarged retroperitoneal lymph nodes, at
1.1cm. The consultant is suspicious it might be metastatic nodal disease. He was diagnosed in June 21 with prostate cancer and psa yesterday is at 0.02 but previous psa in April 2025 was 0.01.
Any guidance or knowledge would be greatly appreciated! Thank you!

  1. Hi . Your concerns are certainly understandable and it is great that your father has you advocating on his behalf and seeking information. Do you mind if I ask what type of treatment your father had for his prostate cancer? I ask because the increase in PSA can mean different things depending on the treatment. Either way. 0.02 is still quite low. In fact, I was going to ask if your father had a PSMA PET scan because it is the gold standard in detecting recurrent prostate cancer (see: https://prostatecancer.net/clinical/psma-imaging-scan), but it is generally considered to only become reliable at around 0.02 PSA. You may want to ask the doctor about this test to determine if any further steps are necessary at this time. Please feel free to keep us posted on how things are going and to ask further questions.     1. thank you! He had radiotherapy and then hormone injections after being disgnoised in June 2021. Bone scan was clear at the that time.
      Is it a concern that his psa was 0.01 last April 2025 to a January 2026 going to 0.02?

  2. No he has not had the PET scan. The letter from the hospital said they were suspicious and that they wanted to discuss his case at a MDM meeting. So still waiting but it feels like torture!

    July 21 commenced Zoladex completed 2 years. Hormone injections.
    October 21 - completed radical radiotherapy to the prostate.

    Dec 22 - psa 0.04
    May 23 - psa 0.02
    Nov 23 - psa 0.02
    Aug 23 - psa 0.01
    Dec 24 - psa 0.02
    April 25 - psa 0.01
    Jan 26 - psa 0.02
