Percent Free PSA and PIRADS 3

Hi all. Im 67, been dealing with high psa’s and low % free psa’s for 10 years. I’ve had 3 unremarkable biopsies during that time. Just had a prostate mri that showed a 17mm lesion that was classified as PIRADS 3. I’m scheduled for a fusion biopsy in one week. Has anyone else had a similar presentation? What are your thoughts?

  1. I should add that my most recent psa was 12.5 and % free psa was 5.73. My psa has been consistently that high for 10 years. % free psa was 7 last year.

      I can't speak to why you have consistently had these high PSA numbers, although there are a number of things that can cause elevated PSA besides PCa. I also need to note that we are not medical professionals and, for your protection, cannot give medical advice. I do want to note that a PIRADs 3 has an intermediate (undetermined) likelihood of being cancer, hence the need for the fusion biopsy. It is possible the results will be negative as before.
      Have your doctors had any thoughts on the elevated levels that previously lead to negative biopsies? Wishing you the best. Richard (Team Member)

      1. Thank you for your response. My doctors have been concerned about my low % free PSA. However, after 3 biopsies I and they have no answers. I have to believe that where’s a little smoke there’s a little fire, but trying not to think too far ahead.

        1. Update. Had the fusion biopsy last week. They took 16 cores, all showed benign prostatic tissue. This leaves me with a couple of questions to ask my doc. Assuming my PSA stays high and % free PSA stays low (I have no reason to suspect this will change) where do we go from here? And, do I still need to take Avodart (been on it for 10 years)? Am I just overthinking this?

          1. Hi . First, let me say congratulations on the negative biopsy results, but the concern and confusion over the consistently high PSA and low free PSA is certainly understandable. I guess a question concerning the Avodart is whether it is helping with symptoms of an enlarged prostate? Also, there is research finding that it reduces the risk of prostate cancer: https://www.nejm.org/doi/full/10.1056/NEJMoa0908127. Do your doctors have any thoughts on the latest results?
