Very Enlarged Prostate

My PSA has been fluctuating for years. I am 64 now.

7 years ago it was 5.65 and then 4.2 -4.7- 5.4-5.1 and now 5.79.
I did the 4k blood test a few years ago and my 4k score was 5.2 ( not psa)
Which was pretty good. I just did an mri and I had one lesion 1.2cm Pl-rads 3. My prostate size is well over 100cc which is extremely large.

Urologist wants to do biopsy. Anyone have a prostate that big on here ?
Is a 1.2cm lesion big ? How is the Pl-Rads score determined…what makes it a 2 or a 3 ? Is it lesion size ? I was a big weightlifter years ago lifting very heavy weight with my legs and just wonder if that contributed to my very large prostate.

I appreciate any and all feedback.

  1. Hi . I can't personally speak to your issue, but, in general, exercise can help prevent or manage prostate enlargement. That said, an enlarged prostate isn't uncommon and the exact causes are unknown.
    Concerning the 4Kscore, I'm assuming the 5.2 is a percentage. One to 7.5 is considered low-risk of aggressive prostate cancer. Note that it is not a test for cancer, just the probability of high risk (see here for more information: https://www.pcmarkers.com/patients/prostate-cancer-screening-tests/4kscore-test.
    A Pi-RAD score on the other hand predicts the probability of clinically significant cancer. A score of 3 falls into the intermediate or basically indeterminate range.     Whether to have the biopsy is your choice, but I hope this information is helpful. Hopefully others will chime in with their thoughts. Best, Richard (Team Member)
