What is ED experience with unilateral nerve spare prostatectomy

I am three weeks post robotic prostatectomy. Due to the cancer location, the surgeon only spared the right side. Pathology showed no cancer in the surgical margins or pelvic lymph nodes so the probability of recurrence is low.

In the lead up to surgery, I was originally planning on external beam radiation treatment but bailed out due to worsening urinary flow issues. Now I pee like a teenager with only minor urinary leakage that is improving. Since I was headed for radiation treatment, I was taking Orgovyx which I stopped the night prior to surgery. I haven't measured yet but my testosterone is still likely low. My question is how have others faired with ED recovery post unilateral nerve spare. I know everyone's experience is different for many different reasons.

I can't personally speak to this, but, not too surprisingly the research finds better outcomes in relation to sexual and urinary comeback for bilateral vs unilateral nerve sparing (see: https://pubmed.ncbi.nlm.nih.gov/21091973/), but potentially better cancer/risk of recurrence with the unilateral depending on the circumstances. I think you hit the nail on the head in noting the different experiences. The prep you have mentioned elsewhere with a pelvic floor therapist certainly can't hurt. Best, Richard (Team Member)
