Rising PSA

First diagnosed with aggressive cancer spread to the bones in 2019when my doctor missed appointments, and before I could see him nine months later, my PSA was at 131. Variousoral treatments, kept it down for a few years, with the help of Elgard. Four months ago I ended my first treatment with Pluvicto and my PSA was at .9. I’ve had no treatment in four months although my PSA has steadily been rising and now it has tripled since last month. It is at 61 last week. I am not scheduled for any doctors appointments until February 17. It is now aggressive and out of control again. Any suggestions? Yes, I have messages into my Oncologist.

  1. Hi . Your concerns and frustration are certainly understandable. According to this Dosing and Administration Guide, Pluvicto is administered every 6 weeks for up to 6 treatments (see: https://geiselmed.dartmouth.edu/radiology/wp-content/uploads/sites/47/2024/09/PLUVICTO-dosing-guide.pdf). Rising PSA after treatment can indicate progression or treatment resistance (see: https://jnm.snmjournals.org/content/63/supplement_2/3062). Of course, we are not medical professionals and cannot say that this is what is occurring in your case. You are always entitled to a second opinion and if you feel you are not being heard and getting the treatment you deserve and need, then you may want to pursue this route.
